If you have previously used BrightStar Care and would like to add Physical Therapy (PT) services to your in-home care, we’re here to help make that happen smoothly.

Discuss Your Needs and Goals
Our care coordinators will talk with you about your current health status, specific physical therapy needs, and goals to ensure we connect you with the right therapist.

1. Schedule an Initial Evaluation
   We will arrange for a licensed physical therapist to visit your home for an initial assessment. This evaluation helps develop a personalized therapy plan tailored just for you.
2. Coordinate Your Care Plan
   Based on the assessment, we will create or update your existing care plan to include physical therapy sessions, coordinating schedules that work best for you.
3. Begin Physical Therapy Services
   Once your plan is in place, a qualified physical therapist will begin visiting your home to provide personalized therapy, helping you improve mobility, strength, and overall well-being.
4. Ongoing Support and Communication
   Our team remains in contact to monitor your progress, address any concerns, and adjust your care plan as needed to ensure you receive the best possible care.
